Ontario virtual care · Ages 12 and older

Virtual care for Toronto residents aged 12 and older

Start with an online assessment about your symptoms and health history. The care team will follow up about your next steps.

Your consultation takes place by voice or video, with call instructions provided once your booking is confirmed.

Virtual-care consultations are free for Ontario residents with a valid Ontario health card. Sick notes are a separate paid service.

We usually respond within the same day, depending on practitioner availability.

Person completing an online health assessment

How it works

1. Tell us about your concern

Choose the concern that best matches your symptoms and complete the online assessment.

2. Receive your next steps

The care team will follow up. Submitting the assessment does not reserve an appointment time. Once your booking is confirmed, you will receive instructions for your call.

3. Speak with the care team

Your consultation takes place by voice or video. Treatment depends on your assessment; a prescription is provided only when clinically appropriate.

Before you start

Virtual care is for patients aged 12 and older. Whether your concern can be managed online depends on the assessment. Some concerns require an in-person examination or another service.

This is an online service for Toronto residents, not a listing for a physical Care365 walk-in clinic in Toronto.

For a medical emergency, call 911. Do not wait for an online response.

Other care resources for Toronto residents

Looking for ongoing primary care?

Ontario’s Health Care Connect information explains how to seek a family doctor, nurse practitioner or primary care team accepting patients in your community. This is a separate service from a Care365 assessment.

Supporting an older adult in Toronto?

The City’s Seniors Health & Home Care Services guide brings together information about health care and support for living at home.

These external resources have their own eligibility and availability. They do not book a Care365 consultation.
